The success of pediatric immunization programs has reduced the over- all burden of preventable infections in children to new lows both in the United States and worldwide. However, immunization of adults does not receive the same priority as immunization of children, despite the fact that deaths from vaccine-preventable diseases occur predominantly in adults. In the United States, 50 000 to 70 000 estimated yearly deaths among adults from pneumococcal infection, influenza, and hepatitis B dwarf the annual mortality from diseases targeted by childhood immunizations (fewer than 500 deaths). Annual deaths from infections preventable by adult immunizations exceed those resulting from automobile accidents or the acquired immunodeficiency syndrome.
Factors contributing to this poor record include lingering doubts of both the public and health care providers about the efficacy and safety of vaccines; uncertainty regarding specific recommendations, concern about liability; inadequate reimbursement; and poorly developed systems for immunization of adults. Among the 19 vaccines provided for adults, insufficient attention is given to setting priorities that emphasize the vaccines that have the greatest potential effect on public health: pneumococcal vac cine, influenza vaccine, and hepatitis B vaccine. Notable preventive medicine successes in recent years have included marked reductions in deaths from cardiovascular disease, lung cancer, and automobile accidents. In each area, the public, the government, and the medical community have collaborated successfully.
